关于病人使用U3-1402的真实反馈及副作用

背景：FDA授予patritumab deruxtecan（代号：U3-1402）突破性治疗称号，用于治疗第三代EGFR-TKI和铂类药物现疾病进展的转移性或局部晚期EGFR突变非小细胞肺癌(NSCLC)。为三代靶向耐药后的NSCLC患者后续用药提供了很好的保障。

关于这个药物的真实情况特节选了几个家庭的反馈，总体来说效果还是很不错的，但是副作用也不小。

Hi all, After about 8 months on Tagrisso (1stline of treatment) I showed progression. I am stage 4, exon 21 (L858R) with mets to lymph nodes, liver and bones. New biopsy did not show any new mutations to target. I am now in the U3-1402 trial that is patritumab deruxtecan in combination with Tagrisso. I’ve had 3 infusions so far and just had my first scans since starting this trial. Very pleased to say that all is “stable”. Questions: Is there anyone else here in this trial?  How long have you been on it? Please share your experiences. Thank you for your responses.

1、My husband is in this trial. He’s stage 4, exon 19, Tagrisso 1st line of treatment, progression after 5 months (11/21 scan) to omentum. He’s had 3 treatments. Results from 2/17/22 scan - decreased omental implants, now 2.7 x 1.3 cm, previously 7.8 x 1.3. Decreased now small to moderate loculated pleural effusion. Slightly decreased small nodule in left upper lung now 1.7 x 1.3 cm previously 2.0 x 1.7 cm.

我丈夫参加了这次入组治疗。他是第4期，第19外显子，一线治疗，5个月后（11/21扫描）进展。他接受了3次U3-1402治疗。结果：2/17/22扫描胸膜转移面积减少，以前是7.8x1.3，现在是2.7x1.3cm。而且局部胸腔积液也减少了。左上肺的小结节略有减少，以前为2.0x1.7cm，现在为1.7x1.3cm。

2、Overall he’s tolerating the treatment well if we could just stay ahead of the side effects. Each week it’s been another “nuisance” - canker sores, severe constipation, tongue sores, severe nighttime leg cramps that we mistook for blood clots (he’s on Xarelto) and ended up in the emergency room, acid reflux/indigestion, nose bleeds, finger & heal cuts.

总的来说，如果副作用是可以忍受得话，效果还是不错得。但是有有严重的口腔溃疡，便秘，舌头溃疡，夜间腿抽筋。

3、Treatment day is a long day, but very tolerable. Day 2 & 3 normal. (He’s 62, very active, very slim.) Day 4 & 5 slight nausea and tired. Day 6 & 7 normal. Weeks 2 & 3 normal, baring the small problems. Magic Mouthwash whenever mouth sores flair up, Pepcid Complete (allowed by the trial) has been effective, antibiotics (dr said common for Tagrisso to cause infection in nose) new skin liquid bandage for finger cuts, stool softeners up to 3x per day as needed, laxative at night, as needed, IV Hydration once or twice during cycle.  He’s yet to get the leg cramps under control. Trying many things, but they’re still a problem.Side note, he had COVID 9/21 (vaccinated 4/21), annoying cough for several days, but that was it. Boosted 12/21.

4、I’ve been in this trial since June 30, 2021. 5 th line of treatment. Exon 21. Nausea is bad for first week-two. Wbc, rdc  and platelets very low.

我（21突变，五线治疗）从2021年6月30日就开始参加这个治疗了。副作用主要是恶心，白细胞、红细胞和血小板都很低。
